These annual forms should be completed by local unit officers and are due January 15, 2026.
All forms are available to fill out and submit ONLINE now. 

2025 Local Officers are encouraged to fill these our in December or early January when its easy to remember the past year's mission projects, events, etc. The financial form should be done with your Treasurer once she has closed the 2025 books.

PAPER FORMS = MORE WORK. You'll need to DOWNLOAD each form, print them, fill them out manually and then mail them to your District President. Then someone else will need to key in your information. 
  If you aren't comfortable online, ask someone who is to bring their laptop to a meeting and do the forms together. It's a lot faster to complete the task that way! Or someone with a computer can ask you the questions over the phone. Work together!

If you do not know your District, then first - go to our DISTRICTS tab and look at the listings there. Hopefully you will be able to determine your District by seeing your city or recognizing the towns near your church. Or call your church and ask them. If that doesn't work, click here and find your church in the list on the left side or on the map. Your District will be displayed along with the name and a picture of the church. Then return to the District tab on this website to get the contact information for your District President.
